CLOSED NOT OPEN SHUT DOWN permanently.

Being shut down by the state. 9/15/19. All residents moved to other facilities. Residents given only 1 week notice, that they will be moved after being at facility for years. Owned by Concord Healthcare Group and multiple other corporations.
