My grandpa who needed oxygen 100% of the time died here! They took the mask off of him and never put it back on again! They only have one person working the night shift it's awful it's all for show here

Too many issues to list in this review, ranging from staff ignoring the patient calling out in the evenings for assistance and neglecting to change dressings twice daily and turn the patient every two hours as ordered by the MD. Although, the most prominent (and deadly) issue was that my grandmother did not receive two of her cardiac meds (that she had been on for 15+ years) for 4-5 days. She was doing great in therapy (which was what she was there for) and walked 30' the day prior to the medication error. Following this med error she started to rapidly decline, and when we asked nursing staff as to why they believed such such rapid decline was occuring, we got a 'shoulder shrug' response. Nursing didn't notice that the two meds had fallen off of her list. It completely went unnoticed until OUR family pulled her medication list and questioned why her Cardizem and Metoporol weren't on it. Lewis blamed the hospital for the manner in which the order was written, but acknowledged their nursing staff should have noticed it dropping off the med list. This was a deadly mistake, as a week later, we were attending her funeral. Watch your family members. Make sure they are getting the care they need. Decent care was provided during the day, but from 10pm to 5am, look out! We even observed staff sleeping during the night shift, specifically on the evening of Feb 26th, while staying with Grandma prior to her death. I spoke with Christine, the Executive Director, a few times throughout her stay, who was very kind, and tried to improve the situation, but she just couldn't get the staff on board and resolve one issue after the next.

Just delivered a pizza to a couple of the most rude and unprofessional workers I've had the displeasure of meeting. God help whoever's relatives they're both taking care of. Beyond frustrating.

Shame on this facility and whoever's is running it.... I have no complaints about the day crew, it's the night crew that are neglectful and rude!My mom has been reprimanded and ignored.... she lost hold of her call button and hollard for help & got yelled out for disturbing the other residents.... when she uses her call button she has to wait OVER and hour & 1/2 for assistance.... she's got 3 broken ribs so she cannot move on her own..... she got pneumonia from their negligence
