Horrible experience very dirty facility very under trained cna nurses who didnt care for there job. We had my mom moved to sedona haven highly recommend Sedona haven. Do not use cottonwood haven for your familys needs.

My mom was admitted and right of the bat, things went wrong... she didn't get her heart medication, which is extremely important and also didn't receive her pain medication when she asked for it 3 different times....Guess I'll be talking with someone in charge very soon....hopefully things will change!

We had a family member in this facility after being in an Excellent Rehabilitation facility in area. This was Sept, Oct, Nov 2019. She was there to continue her therapy following a stroke. That left her unable to speak but fully aware & mentally intact. I visited her there many times & left each time very depressed & angry. The care she received there was basic if even that. They DO NOT have adequate staff there to provide care for the people there. If she had been there even a wk longer she would not be alive today. She left there dangerously dehydrated, running a relatively high fever, several bedsores infected finger. When visiting several times trips were needed to Bathrm & I ended up taking her myself as no staff to be found. This after call buzzer rang many times. I am a former nurse & have worked in Many skilled nursing facilities doing private care. After meals the wheelchair bound Pts were just taken to area in front of an empty nurses station, wheelchair brakes locked & then patients just left there for hours at a time. This happened to someone who had people that visited & checked up on her & the staff knew that. I hate to think of the patients that don't have that same type of monitoring their well being & living out their final days. It breaks my heart.

Transferred here on 12/23/2019 from the ER. I slept 10 hrs first night because I had not slept during the previous 28hrs at ER. The hospital staff recommended I come here because I lived alone and they wanted me well before going home. 'Holidays' -not properly oriented to the facility on first day; did not receive my normal maintenance meds until 3rd day; food unpalitable; yes, received physical therapy promptly 2xday, whether i was slept, which i could not. I told them I suffered from insomnia and needed otc melatonin for this, hey finally received the 3/4 night, but not the correct dosage, so no sleep for me 4nights. I left the facility AMA on 12/28 as soon as the sun was up. Observations: staff not happy, staff disrespecting patients/inmates; 99.9% patients in wheelchairs; patients not eating the food; 4 inch foam bed mattresses covered in plastic; itchy blankets; room temps 77-80 degrees, i.e. uncomfortable room environment, where of course a patient will stay in 99%of the time. Yes you are assigned a day for shower, there are activities, a library and several rooms to receive your Loved Ones. Luckily my roommate had been there a long time and quite mobile with her wheelchair, so I learned more from her about the facility than from the staff. If I was recovering from an injury/surgery I would stay away from this place. It is more for those who have memory impairment. PS I am a former Certified Nurses Aide and trained /worked in these facilities,

I don't want to come back, but if I do have to, I will insist on Haven of Cottonwood.
