I am not sure where to even begin with our horrid experience here. My husband was admitted here for rehab after an accident in our home. He suffered pretty significant painful injuries that required surgical repair. While hospitalized, a cardiac condition was discovered and another surgery was performed to fix that issue. He was walking fairly independently upon being admitted, but that quickly changed. His blood sugar upon being admitted was extremely high. The nursing staff did nothing to being down this dangerously high blood sugar. They did not even seem concerned. In fact, the staff brought him his carb laden dinner whilst his glucose was at a very scary unsafe level. Still they did nothing. I immediately asked for a transfer from the facility to a different rehab. The next day, we requested a meeting with the social worker, Mary Jo, the Director of Nursing (I cannot remember her name) and the interim administrator, Dominique. I had to get to work and would return to discuss our concerns at noon. I returned a noon, none of the three ladies in sight. I had to track down the SW and DON to come in, the interim admin decided it wasn't important enough to keep her word and was out of the building. During this meeting, my husband was repeatedly threatened that if he refused treatment or refused to do things the way they wanted them done, then he would be marked as refusing rehab and we would have to pay privately for his stay. I got upset and tried to explain our concerns regarding diabetic and pain management. I was told to shut my mouth and instructed to leave the room by the SW. We were very upset by the way each of us was spoken to. Each staff member be it a CNA, Dietary, PT/OT or LPN talked to him like he was a three year old child, They refused to allow family to participate in caring for him, as my husband preferred. I was physically pushed out of the way by and aide name Linda while ambulating with my husband tot he bathroom. I was told I had no clue what I was doing and that only she could assist. My husband quite plainly said he did not want her assistance, but his wife to assist him to the restroom. The next morning, a weekend, he was incorrectly yanked up in bed by his left arm, violating his post surgical precautions, then yelled at when he had to get upset and order the aide to take her hands off of him. PT refused to work with him because they stated 'Well you are being discharged to another facility, so what is the point of working with you.' They demanded to have 2 CNA's storm into his room like they were storming the shores of Normandy every time he pushed the call light. Even though the care plan taped in his closet clearly stated a one assist or independent transfer. I attempted to give him a bed bath and they tried to prevent me from doing so, stating only they were permitted to do so. He verbalized he wanted me, his wife, to do the bath. It was while giving him a bath that i noticed his abdomen was painfully distended. He had not had a BM for a few days and I encouraged him to get up and walk. We were going to walk in the hall, only the aides came in and said that he was not allowed outside of his room by PT. PT had to 'sign off' on him being outside of his room....but they refused to work with him. He was restricted to his room for two full days illegally. HE began to develop Grade 3-4 edema on both legs. When I pointed it out to the nurse, she snapped back saying it was his fault because he did not put his compression hose on until late.....the nurse put them on....BACKWARDS. The CNA's would not give him privacy while using the restroom despite repeated request to do so. On Sunday he was extremely lethargic to which they refused to give his pain medication. Turns out, he was severely anemic. All this time we were waiting for a transfer. The Social Worker, Mary Jo, took it upon herself to skew the referrals by slandering me as a difficult family member. If expecting my loved one receive top notch care makes me 'difficult' then so be it! STAY AWAY FROM HERE

After over two weeks in the hospital, Sun Prairie's PT and OT and nurses nursed my 86 year old dad back to health. He said he felt safe there and he enjoyed participating in their organized activities. I would recommend as a rehab facility: It looks bare bones, but good staff.

Very Unprofessional. Almost every person , any position were very rude. Dietary staff never showed up. Very negative environment, poor management. Only positive about this place were the residents. Dont recommend this nursing home to families.
