Before you pay a $2500 community fee, be sure that you have a full understanding of the tiered care/billing system at WSMC. If not provided, insist upon a signed, dated and fully filled out copy of your contract. Ask specific questions regarding the criteria for each level of care, including end of life/palliative care. Mosaic Management's newest administrator (11/2018) informed us that WSMC could no longer meet our Mom's care needs. Think through what you will do and where you will go in the Corvallis area if the natural progression of Alzheimer's disease brings your loved one to a place of needing assistance with feeding or a two person transfer. Direct care staff is stretched thin by the physical size of this large, four quadrant 72 bed facility. Scope of this issue is evident when you review the public record of state licensing violations for 2017, 2018. No matter which memory care place you chose, know that you are entitled to a full disclosure of Oregon Resident Rights.

The people that work there are very caring and you can tell they all love what they do. The Executive Director is so knowledgeable and friendly. Such a beautiful community to place your loved one.
