My mother was a resident from Oct. 2018 to Nov. 2019. There was a complete overturn in management and nursing staff. Furnishings were becoming dirty and worn, often seats were wet. During Oct. 2019 my mother was left unchecked the entire night on 3 different nights. Management initially refused to release me from my contract 30 days from notice. My mother on hospice. I requested the day I told the ED. I was looking for another place for my mother. It has been 9 weeks and we are are still waiting for the refund of the month rent. I've tried 4-5 times to call corporate but have only reached a call center in NC and no one has returned my calls.

(written by Anne Boxeur) My 98-year-old mother-in-law has been a resident at Autumn Leaves and they have worked wonders with her! Their Director of Nursing, Mary Kay Trotter, is the most compassionate and dedicated person you will, ever, find in a patient care community! When my MIL, first, moved there she did not interact socially, at all. Through proper stimulation and environment, she has really come out of her shell and we owe her new-found independence to the opportunities that she has been given at Autumn Leaves. Jonna Warrick, Executive Director, works very hard to keep a lively, upbeat atmosphere - planning fun and meaningful activities for the residents. If you are looking for a total memory care community, please consider Autumn Leaves of Edmond!!

Autumn Leaves and Denise Diaz are a class act. Their caring is above and beyond, authentic, and from the heart. Highly recommend for compassionate care of loved ones with cognitive impairment.
