Centerville Health and Rehab

Resident's refrigerator is malfunctioning and has not been fixed. It has been a week. Meds not given on time. Sometimes 3 hrs. Late. Report to Ombuspersons need to ge made about this place.

Allowing my mom to go to Centerville Health and Rehab was a horrible mistake! I honestly cant think of one positive thing to say about this place. It is not clean, they are understaffed, the nurses are not attentive, and the food is pathetic. The social worker never returned one phone call. Due to an abrupt discharge, we were unable to retrieve her laundry. After following up by phone and in person for over 2 months, we never saw her clothes again! We have been to several other short term rehab facilities in the area over the past few years and this was by far the worst! Please do not trust in the care of your loved ones to Centerville Health & Rehab.

I wouldnt recommend working for this place let alone have your loved ones there.. state is in there every week.. the place has rats and theres no staff... my first day of orientation there was one aide to 60 residents... they have no administrator or a director of nursing.... I was fired over the phone today 2/4/2020 basically because I wouldnt take an assignment on my very first day of orientation.. the residents are not getting the care that they pay for... I will be contacting Ohio department of health to do a full on investigation about the care that residents receive... the building I ran mostly by agency staff. I feel as though I was discriminated against because the Caucasian staff made up lies on me and I was terminated (over the phone) unprofessional the place just needs to be closed down. And Im going to help do that.... BEWARE IF U LOVE UR FAMILY MEMBERS DONT SEND THEM HERE OR IF U DO MAKE SURE U HAVE CAMERAS UP IN THEIR ROOM,,,,
