My Mother Sandra was in Chapel Hill Nursing home from approximately the 17th of December through the 31st.I began to see many red flags from the beginning starting with orders for TED hose not being implemented in a timely manner. It took them several days to get them on her, and I have knowledge from a friend that this is an item stocked at most nursing homes. My Mothers pain was not being managed in any way. She is a stage 4 lung cancer patient that has been on several pain killers throughout the years so her tolerance is a bit higher than your average geriatric client. With this being said she was only receiving on Percocet every 4 hours. She was unable to participate in PT due to her constant pain, and nursing staff still didn't see an issue with this. I attempted to reach the DON on several occasions, however when transferred to her office her line went straight to VM, and her VM was full. I then called the Administrator and voiced my concerns with her concerning my Mom's inability to do PT due to her pain, and how I felt like she was not getting proper care, and I would be better off taking her to my house. The administrator responded very professionally to this by telling me that they could get the paperwork started instead of addressing my concerns. I contacted an Ombudsman at this point. Several other issues were the fact that my Mom was not showered after having been at their facility for 9 days. She lost close to twenty pounds in the two week period she was there, however the Ombudsman said they were marking 100% for all of her meals. The last straw was when I contacted my Mother New Years Eve, and she informed me that she had fallen in the bathroom the night before and hit her head, and layed in her own urine yelling until somebody finally assisted her up. At no point did nursing staff contact me, or anyone else in her family. They did however shortly after learning that the family was aware decide that it would be a good idea to send her out for a head CT. After that point my Mom never returned to Chapel Hill, and I feel certain that if she did she would have died there soon after.

My mom went here to rehab from a broken rib on December 16th. On December 31st after having lost 21 pounds, developing wrist drop, and pneumonia, she fell in her bathroom and broke 5 more ribs. She laid in her own. urine for several hours calling for help. She was bleeding from her head. At 2pm in the afternoon they thought it was a good idea to send for a CT of the head. In between I went there to inquire about her 21lb weight loss and why it took almost two weeks to get a wrist splint. The DON Phana was the rudest most uncompassionate person I have ever met. Basically told me that sometimes it takes two weeks to get a wrist splint. Didn't apologize at all. Then told me that despite her MDS nurse Chris Greenfield telling me my mom had lost 21 lbs and her NP telling me my mom had lost 21lbs, that in fact my mom had only lost ten pounds. in 14 days. Then her assistant told her that I wasn't a 'responsible party'. At that point she told me to leave. 3 or 4 hours earlier I was on the phone with her and her nurse when my mom gave permission for me to get info. When I told her this she said she would call the police if I didn't leave. I never once cussed at her or raised my voice. I have never felt so disrespected. There are other things like not having a clock or calendar in her room. O2 tubing on the floor, call light not within reach, and garbage food. But these seem like small potatoes. Also, nobody calls you back unless you call in multiple times. I was sent to a voicemail where nobody identified themselves. My sister was sent to the DONs voicemail, which was full. And before I sat down with the DON I heard her telling staff that she works so hard she doesn't even have time to take a piss. Super professional. Make your own choice. My mom will not be back. They did a good job of most likely hastening her life. Thanks Phana and Chris Greenfield for all your help. I totally forgot to mention they called the cops on me and the DON told me to 'get the hell out'. I'm sure they will counter with some BS response but I have no reason not to speak the truth..... Update. At one point when talking to the DON Phana she denied the weight loss. However in the ICU in the hospital the weight loss was confirmed at 18lbs in 2 weeks at this nursing home. We also were not called after she fell. And they waited at least 8 hours after falling to send her to the hospital. I also forgot to mention that i am my mother's POA which further infuriates me that they would tell me that i could not get info about my own mother. We also had a nurse tell us that my mom should be hospice care. She said this right in front of my mother. She probably would have died of she stayed in that nursing home and that is not hyperbole.

My mother was a resident there in fast track rehab. She got great care from the nurses, aides and rehab staff. She looked forward to having dinner in the dining room every evening with my dad. She even became friends with some staff members. Unfortunately, she did pass away, however the way the staff cared for her and our family was truly remarkable. This community added abundant life to my moms final months on this earth. Thank you for your ministry.

I can only say this place and the director of nursing are disgraceful. My mother in law would have died there had we not constantly checked in. Checking in one day we find out she fell in the bathroom, cut here head, broke her ribs and laid in urine for 20 some minutes screaming. The food is disgusting, the place is dirty and the place is understaffed. The Administrator does not care either. Sandy was sent to the hospital where she remains as will not send her back to Chapel Hill. She deteriorated so rapidly the 2 weeks she was there. I would never ever recommend sending your family there. This place is a disgrace.
