Very disappointed in the employees at this facility. They talk about dignity of the patient yet they will drug them into a stupor in lieu of putting a lap belt on them to keep them from falling. Poor management results in chronic poor care among certain employees. Meals were forgotten multiple times on weekends, Patient fell out of wheelchair several times with no alternate prevention plan until we got state involved. Staff caught laughing at dementia patients. Dementia patients pretty much warehoused and drugged if family members don't keep tabs on things. Better have an advocate close at hand to make frequent stop ins. They've recently changed the name of facility but still same staffing issues. Be aware. State investigation has been made.

They dont give the residents NO SNACKS!!! They let them go hungry The DON heather she was cursing one of the residents. worst facility will be calling the state !!! I hope they shut down this nasty building. They use all the residents funds for thereselfs and Limit residents. They dont give the residents baths never there skin is scale due to no bathing. They dont check on residents

I used to volunteer there. They are horrible to the residents. They dont help feed the ones that have trouble feeding themselves.

Mom is in Ocean Trail Rehab facility. She is receiving excellent care. Nursing, physical therapy and CNA staff is very efficient, responsive and caring. They are attentive to her needs and her privacy. The new rooms are excellent. I had to move mom from another facility; so, I have a comparison. Do not be concerned on the low rating. I can find no fault with facility or care. Highly recommend
