I wish I could rate this place zero stars! I have a lot to say and I'm not sure where to start. First and foremost, I hope and pray that the owners of this facility (Wellbrooke) reads and will feel some sort of responsibility in the neglect that was given to my mother, who has since, passed away. Let me introduce to you some information needed to understand why this facility was needed for my mother's rehab/physical therapy. My mother of 72 years, having slight Parkinson's disease, needed surgery on her upper vertebrae to relieve symptoms she was suffering. To get to the location on the upper vertebrae, doctors had to go through the throat. Going through the throat means she would need rehab/ physical therapy after the surgery to regain strength in her muscles that assist in swollowing liquids/food. Previously, this facility was chosen by my parents.Once my mother was admitted into this facility for her much needed physical therapy, our lives and especially my mother's would eventually be forever changed. Between my dad, sister(RN) and myself, we took turns in seeing/visiting my mother while on her temporary stay here. This facility knew about her being diabetic and was needing to be on a diabetic diet. I had witnessed numerous times my mother being given foods that did not follow the diabetic plan. This issue was brought to the attention of staff and later my dad informed the Manager. Afterwards, it again was witnessed that the diabetic plan was not implemented 100%. My mother was slowly being malnourished at every meal since there was very little choices on her plate of what she could eat. The facility had documented routine sugar level checks which actually showed very high readings. Insulin was administered however, the facility wasn't giving the correct insulin dosage amounts which was not helping my mother's levels to come down to normal range. My dad had previously informed the facility and doctor what the actual formula was for my mother's insulin dosages. I can tell you that it was numerous times of us all finding out that the staff was not administering the correct units of insulin. Now, she is being malnourished and they are failing to keep her sugar levels within normal range. On another visit by me, I witnessed my mother's liquid intake not being monitored accurately which in turn made her start to dehydrate! The main reason for her being there was because of not being able to swollow that well and here she is not getting the proper liquids or food! On the same visit, my mother was telling me that she couldn't lift her head up very well because it was stiff and hurting. I informed the nursing aid that happened to be in the room at the time in hopes that she would notify a nurse and come up with a solution. Well, the nursing aid's solution was to forcefully move my mom's head to the upward position! I was livid! My mom's neck had been through surgery and this nursing aid lacked apathy towards her! This all was reported and long story short, mom was sent to the hospital three times from this facility and the last time sent to the hospital the staff there found severe dehydration, severely low blood pressure, C-Diff (which she became septic), sugar levels high, UTI, etc.!After some time, my mother never recovered and passed away. My only mother, who was there to simply get physical therapy and then go home, was taken prematurely from us due to the continuous neglect she received.I will always see this place as a 'one way ticket'.

A bright cheery place without the smell of a nursing home/Rehab facility. Food is the reason for less than a 5. While nutritious it is very institutional in preparation. The Rehab staff is a 6, outstanding, simply outstanding. Grounds are pleasing and most if not all rooms have windows. There could be more outdoor space with shade. The staff quickly learns your name. Has to be the best place in town.

If my loved one needed rehab, Assisted Living, or skilled nursing care they would be at Wellbrooke hands down! Great place to live and work! The campus is clean, the food is tasty, the activities are engaging for both residents and family, and the clinical care is on point. I highly recommend Wellbrooke of Kokomo!
