Despite the distance from Mansfield to Laporte, it is a very nice, well-kept. They seem to be very on top of things with my grandmother. While she has fallen a few times while in their care, it is not out of their attempted prevention. She is constantly monitored and on bed alarms.

My husband had fallen 4 times while in their care within less than a week.Treatment and Case Management teams are incompetent and he died while in their care.
