My parents moved into Markham House (10606 SW Capitol Highway, Portland, Oregon) in October 2014 when they could no longer live on their own. At that time, it was one of the highest rated assisted living facilities in Portland and the rating was well deserved. My brother, sister and I visited often and closely communicated and worked with the staff to make sure our parents were well cared for, safe and treated with dignity. All was well until mid-2018 when things started to deteriorate significantly. Our mother passed away before the decline but our father remained until his death in July 2019. During that last year, there were a number of incidents that caused us to question his safety, well-being and dignity. We raised many of these incidents with the facilities General Manager and staff members as they occurred. We seriously considered moving him to another facility but our concern for the effect a move would have on his mental capacity resulted in a decision to stay. Following his death, we sent a letter to the CEO of Leisure Care, the parent company of Markham House, detailing our observations and concerns and providing a number of examples of the incidents we experienced. We also copied the State of Oregon. The Operations Manager of Leisure Care contacted us to advise us they were taking our letter seriously and were reviewing and monitoring the conditions. Oregon Adult Protective Services also contacted us to investigate an incident whereby our father did not receive some prescribed arthritis medication for a period of weeks. As a result of our experiences and despite the excellent care for the first four years, we do not recommend Markham House for retirement or assisted living services.

I read the last few comments, I would say that an outsider looking in and observing those comments and concerns are pretty valid! Thank you Suzzane for mentioning your comments; I couldnt agree more with you! Markham House had a really great HW Director and RN that cared for the residents. Those are the type of individuals that should be in charge of running a place like Markham House, especially knowing that my loved one as well as others were not happy because their concerns were not being heard. The Previous HW department leaders made every resident feel important. Thats something you will never see in this building again. You currently have a building manager thats rude and condescending and talks down to staff in front of family members and residents. Thats not right! To add I seen in a below comment that were supposed to contact Holli for further issues, well your current manager of the building do not talk high of Holli and it sounds like the building manager and Holli cannot be trusted. They both seem to be involved in pushing the last HW Director and RN our the door. Im very disappointed with Markham House.

To whom it may concern:My mother has lived at Markham house for a couple of years now. Since she has been there it has been a roller coaster with trying tokeep my mother happy as well as the other residents from what I encountered. Im bringing this up because, recently this facility hadtwo great amazing nurses that were very involved In The residents daily lives. One in particular was extremely good at what he does and he was alwaysconstantly working on the floor every time that I seen him. Any time that I had an issue or that I noticed anybody else had an issue wewere able to grab him at any time and he would focus his attention on the resident or that need at that moment. He did all of this whilenot having an assistant and only having one other amazing nurse one day a week. This is very disturbing to me because I had went on vacation recently and I wanted to come back to thank him personally for all of his hard work and that I noticed that he had been very attentive when it comes up to the residents and my mother specifically adored that about him. Amongst my research it came to my attention that this company and him got into a disagreement and instead of being a support system for him and helping him when he needed help like he did for all of his employees and staff they painted him out as if he was not great. He was a hard worker and he was always on the floor and he was always helping someone every day that I was there! This company will never find two great nurses that will work on the floor and make patients their priority. He was encouraged to do a lot of things that were not appropriate in many ways, instead of working with him, they thought it was easier to force him out of the position. My family and I are very devastated because we recently also found out that his co-partner ended up leaving due to the non-sense thatexist in this building and how this company had treated The Former Director, and prior directors and nurses. The president of this company needs to re- evaluate whoever he has in charge in know that stuff like this is not right and need to figure out how to make it right. I want allfuture families to read this and know what they are getting their self into and what kind of company this is. For how this companyhandled a few situations over a last couple years this was the straw that broke the camels back. My personal ratings would be a 1.5stars! They need to bring back the most current HW director and get him an assistant and they need to bring back his partner nurse that worked one day a week. Things are going to get out of control, and them two together were great at putting out dilemmas. No respect for this company at all!

There is a handful of staff there that I enjoy! I have noticed over the last year or maybe a little over a year, I have noticed a dramatic decrease in employee satisfaction and huge resident safety concerns. I have heard myself a lot of conversations from residents themselves regarding information about the General manager Zohreh Jamshidi. Residents are aware of firings within the company that has happened recently. One story In particularly that was told to my grandmother at the dinner table was that a former employee had gotten fired for following the policies and this employee spoke up and notified the family being open and honestly about it and the General manager retaliated against this employee by firing this individual. Later on at that same dinner table the resident and the residents family happened to be at the same table and provided more disturbing details validating the story. In this exact night I witnessed myself that Zohreh the general manager talking in very condescending way in the front lobby toward another employee. This is a huge concern to me and it has been brought to corporate as well as the states attention! We as families expect for our loved ones to be taken care of and told the truth when bringing our family into places like this and for this one particular family to have gone through so many lies and treated that way has me second guessing my grandmothers stay at Markham house. This is has become a a safety issue for not only the residents but it seems like a hostile working environment for employees too! Something seriously needs to change!
