Elmore Assisted Living & Memory Care

I have been working with Stephanie from Sunflower Communities for well over a year now- it was such a relief to learn about their facilities as many of my clients are more difficult to place because they tend to have a lot of physical and mental health needs- I have clients in both their Elmore and Waconia locations and they do an excellent job!

To date we have several clients living at both Elmore and Nagel Assisted Living & Memory Care, all of them are well taken care of and thriving. Had it not been for their open mindedness and willingness to get to know our clients these individuals would have most likely been prematurely placed in nursing homes. Thank-you Stephanie for coming in to our case management offices to educate us on the excellent housing services you offer and continuing to update us on availability- we all appreciate it!

As a Hennepin County Case Manger working with individuals with all sorts of behavioral health needs its refreshing to know that communities like Nagel and Elmore Assisted Living & Memory Care really do exist. Working with Stephanie has been a joy!

What I like best about working with Stephanie Bauman from Sunflower Communities is her honesty and commitment in helping the people we serve. She has done home visits with my clients to answer questions, address concerns and share pictures of the community with them- can you imagine how stressful it would be to move in to a new home without being able to see it- Stephanie is great at helping to ease their fears. In the 9 months I have been working with Hennepin & Dakota County I have successfully placed a handful of people at both Nagel and Elmore Assisted Living and Memory Care.

When I first met Stephanie Bauman from Sunflower Communities I have to say I was hesitant to believe all the things she was telling us- Assisted Living and Memory Care Communities that don't shy away from individuals with behavioral health needs but more importantly communities where every rooms is eligible for CADI and EW waivers. As a county case manager, all of my clients are on waivers and some of them are harder to place then others because of their mental health needs. It only took me to refer to her once for me to realize that everything she told me was true to include their stellar communication. Honestly I have worked with both buildings (Nagel & Elmore Assisted Living and Memory Care) and I feel confident referring to them because I believe they offer a great housing option to those with high care needs to include behavioral health but also because in communicating with Janelle, Eva and Stephanie- when they say they will call back they do. And last but not least, I would like to add that my most recent referral moved from the Northern suburbs of the twin Cities to their community in Southern MN (Elmore) and I was shocked when they told me they would move my client from their home to the community to include their personal belongings... who does this? THEY DO!
